The Awakening Master text is updated. Live after NPCs are reloaded (or the next restart).

Awakening has always been able to break gear. If the upgrade fails, the item is destroyed. Materials and gold are still spent. The NPC did not say that clearly. It does now. What you will see

• The Awakening Master warns on the main page that a fail destroys the item.
• Awaken opens a short warning page first. Continue opens the upgrade panel. Back returns to the menu.
• Disassemble also notes that it destroys the item.

You can awaken an item up to 5 times.
